Client Health Questionnaire

With the Ontario Government announcing that Hair Salons are apart of the Phase 2 re-opening, we want to ensure that our studio and our clients remain healthy and safe. If you have an appointment booked, please ensure the below health questionnaire is filled out.

Please note, if you are feeling ill or are taking care of someone who is ill, we kindly ask that you reschedule your appointment.

PRIOR TO THE START OF MY SERVICE I CONFIRM THAT:

I have not been diagnosed with or cared for someone who has been diagnosed with COVID-19 in the past 2 weeks

I have not shown symptoms of COVID-19 or come into close contact with anyone exhibiting these symptoms in the past 2 weeks

I do not have a cough, fever, chills, shortness of breath, or loss of taste or smell

If I begin to show symptoms of COVID-19 within the next 2 weeks, I will contact my stylist

I will follow all posted salon rules to keep myself, my stylist and those around me safe
